ISSWSH remains fully committed to holding a successful in-person Annual Meeting at the Renaissance Dallas Hotel, Dallas, TX in March 2022. The priority of ISSWSH remains the health and well-being of our community. Due to the continuing prevalence of COVID-19, as a registrant and attendee of the 2022 ISSWSH Annual Meeting, I understand and agree to the following:

1. My attendance at the 2022 ISSWSH Annual Meeting requires additional measures and policies that have been instituted by ISSWSH to mitigate the risk of infection and the potential health consequences associated with COVID-19.

2. I agree to provide proof of one of the following:
   -  I have been fully vaccinated prior to attending the 2022 ISSWSH Annual Meeting and will provide proof of vaccination status.
   -  I am not fully vaccinated and will obtain and show proof upon arrival of a negative PCR coronavirus test within 72 hours of the first day at the 2022 ISSWSH Annual Meeting.
3. I will refrain from attending the 2022 ISSWSH Annual Meeting if I am exhibiting any of the following symptoms that the CDC recognizes as being associated with COVID-19, including but not limited to, fever, cough, nasal congestion, runny nose, nausea, vomiting, diarrhea, shortness of breath or difficulty breathing, chills, muscle pain, headache, sore throat, or new loss of taste or smell.
4. I will be required to wear a face covering when indoors at all 2022 ISSWSH Annual Meeting activities.
5. Individuals with COVID-19 can be asymptomatic and unable to self-diagnose or otherwise unable to disclose the potential for infecting others. Thus, I may be exposed to individuals with COVID-19.
6. I understand that any information relating to COVID-19, including this confidential declaration, will be safeguarded by ISSWSH and protected from unauthorized uses.
7. I understand that ISSWSH must follow Federal, State, and Local laws which may change from time to time.
